To receive your Nevada medical marijuana card, patients must download and complete the Application Request Form from the Division of Public and Behavioral Health. Before you may obtain a medical marijuana card, you must first obtain a recommendation from a licensed physician for medical cannabis.

Download and complete the “Application Request Form” from the Division of Public and Behavioral Health:
Mail-in with Fee
Mail the completed form with a check or money order for $25 made payable to the Division of Public and Behavioral Health to the following address:

Application Kit & Doctor Visit
If your form is successfully processed, you will receive an Application Kit in return. Upon receipt of the Application Kit, make an appointment with a qualified physician to obtain a recommendation (not prescription) for medical cannabis. You can go to your standard doctor or a clinic specializing in medical cannabis recommendations.
Mail-in Application
After completing and notarizing the Application Kit, you must mail it with a $75 check or money order made payable to the Division of Public and Behavioral Health. Mail the kit and the check to the following address:

Temporary Approval
Once your application has been successfully processed, you will receive a temporary 30-day approval letter after the background check is finished. You can shop at dispensaries as a recreational customer while you wait for your application’s approval.
Approved Status
When you become fully approved as a medical cannabis patient, an acceptance letter will be sent to your address on file. Take this letter to your local DMV to receive your program identification card.
